About the job

We’re looking to recruit enthusiastic, experienced and committed Care Managers to our Access and Assessment Service. Utilising your knowledge of working with adults, predominately Older people, you will be expected to undertake assessments of need, and support the implementation of personalisation to develop creative careplans to meet agreed outcomes. We have developed a range of resources to enable the residents of Hillingdon to remain in their own homes, underpinned by the philosophy of independence and choice.

About the person

In addition to your Social Work or other appropriate qualification, you’ll be registered with a relevant professional body. You’ll also have experience of working within an Assessment and Care Management environment, which will give you the understanding, knowledge and capability to undertake assessments, manage casework, and understand, interpret and apply relevant Community Care legislation. It will be essential that you have well developed interpersonal skills, and the ability to work well under pressure and within a team. You must also be well organised to know how and when to act independently.
