The post is required for the Locality Learning Disability Team in Dartford, Gravesham and Swanley based in Joynes House, Gravesend.

The team provides multidisciplinary support to adults with a learning disability. A relevant professional qualification and current registration is essential ie Social Work, LD Nurse, OT.

The successful candidate will have drive and initiative and the ability to use a person centred approach to assist and support clients to identify eligible needs, assess, monitor and review the support/personal assistance needs of service users. Candidates should be committed to working in partnership with service users, carers and provider agencies to deliver our personalisation agenda and the objectives of Valuing People Now. You will also have proven experience of working with people with a learning disability. All staff are accountable and responsible for their practice and allocation of resources. An ability to manage change is needed due to the ongoing restructuring of Kent County Council.

The ability to travel across a wide geographical area in a timely and flexible manner at various times of the day is essential.
