We have recently introduced a new Access, Assessment and Care Management structure to give people the opportunity and flexibility to achieve a truly personalised approach to the support they receive, and our employees the ability to demonstrate innovation and true partnership working, with a clear commitment to their continued personal development.

This is an opportunity to join a developing, multi-disciplinary team, based at Walsall Manor Hospital. As an advanced practitioner you will be able to develop the practice of the team, in ensuring that discharges from hospital are facilitated in a timely manner. The team are already one of the highest performers in the Midlands Region.

With at least four years post qualification experience, we are looking to fill these vacancies with enthusiastic people who have the appropriate skills and qualities to join our existing committed team members to deliver personalised services.
